Hemovigilance is a series of monitoring procedures that encompass the entire transfusion chain, from the collection of blood and blood components to the follow-up of recipients, with the aim of collecting, evaluating, and preventing unexpected or undesirable situations arising from the collection and clinical use of blood and blood components. In our unit, a hemovigilance team was established in September 2018 to ensure the quality and reliable delivery of blood and blood products services and it is actively carrying out its activities. In early 2019, the "Hospital Blood Management" algorithm was defined as a set of medical and surgical approaches aimed at maintaining and protecting the patient's own blood at a sufficient level to eliminate or reduce the need for transfusion. This algorithm has been implemented actively in the field.

Duties:

In accordance with the 2016 National Blood and Blood Products Guide, our unit performs blood typing, cross-matching, and other tests (indirect Coombs, direct Coombs, subgroup, antibody screening, and identification) when necessary. We obtain blood and blood products from the North Marmara Red Crescent Regional Blood Center. (Washed red blood cell suspension, irradiated red blood cell suspension, and platelet suspension are provided when indicated.) In necessary cases, with authorization from the North Marmara Red Crescent Regional Blood Center, we perform fresh whole blood donations. We track the expiration dates of blood and blood products. We monitor the critical stock level. We keep stock records of blood and blood products received from the Regional Blood Center. We handle the disposal of blood and blood products. Official documents to be preserved (donor inquiry forms, transfusion request forms, rejected donor inquiry forms, etc.) are archived. We conduct daily monitoring of donor tests. Hemovigilance covers all adverse reactions that occur in blood donors or recipients and all adverse events that occur in the transfusion chain. Additionally, epidemiological surveillance of blood donors is ensured. The main goal of hemovigilance is to increase the safety of both blood donors and recipients (transfusion) by preventing the recurrence of adverse events and reactions. We access reliable information about adverse events and reactions related to blood donation or transfusion. We take corrective actions to prevent the recurrence of adverse events and reactions as a result of errors during blood donation and transfusion processes. We alert the hospital and blood service units to the fact that adverse events and reactions may affect many people. (Transmission of infectious diseases, errors related to medical devices, materials, equipment used during blood donation or transfusion, etc.) We monitor whether all transfusions performed in the hospital are properly documented with the Transfusion Monitoring Form. Periodic training sessions are organized in this regard. These forms are also archived. In case of suspicion of a transfusion-related reaction in the recipient or the identification of a situation threatening the safety of transfusion in the donor, we track the current status (transfused patients, production, destruction, or stock status, etc.) of blood components obtained from this donor. We create the hospital's annual hemovigilance reports. (Annual Notification Form for Adverse Events, Annual Notification Form for Transfusion-Related Adverse Reactions)
